for individuals ages 13+ with no history/presence of periodontal disease

Your In-House Dental Plan is active for one year from date of purchase and renews every 12 months from purchase.
Your In-House Dental Plan yearly fee is non-refundable.
You will be charged $50 for missed appointments or “no shows,” or you may choose to forfeit one exam and cleaning in lieu of the $50 charge. No charge or forfeiture for appointments cancelled or rescheduled 24 hours in advance.
Your In-House Dental Plans may be cancelled without a refund for frequent cancellations, reschedules, or “no shows.”
In-house plans cannot be combined with any other special offers, discounts, insurance, or third-party reimbursement.
Your In-House Dental Plan is not valid at other dental offices or referred specialists.
Your In-House Dental Plan is not insurance. It is a discount through Walls Family Dentistry.
